Where to get the exit hood kit

Regarding Page 140 of the book ‘Final Exit’ concerning the Exit Bag, better known nowadays as the ‘helium hood kit’: The ordering detail no longer applies.
Last Rights Publications in Canada ceased in 2004 due to police harassment. Usually people who send there get their money back – eventually – because the P O box is closed.
